Hospice care focuses on quality of life and symptom
management, allowing individuals to stay at home
or a facility. A team of healthcare professionals addresses
​physical, emotional, and spiritual needs, with 
family involvement in care decisions.

Routine Hospice Care

Hospice routine home care is typically recommended when the patient is no longer responding to curative treatments and has a prognosis of six months or less. Hospice care focuses on helping patients and families make the most of their time together, managing pain and other symptoms, so the patient can live as comfortably as possible.

Continuous Care

Continuous Care Is a higher level of care defined by the need for a bedside nurse for acute symptom management.  If a patient is living at home, a board and care or an assisted living continuous Care is provided when a patient is having an acute symptom management crisis and requires immediate nursing attention for an extended period of time.
