Job Description

The Medical Social Worker (MSW) provides essential psychosocial support to patients and their families facing terminal illnesses. This role involves assessing the social, emotional, and financial needs of patients, facilitating access to resources, and helping families navigate the complexities of end-of-life care. The Medical Social Worker collaborates closely with the interdisciplinary team to ensure comprehensive care and support.

Qualifications

• Graduate of a bachelor's program in social work accredited by the Council on Social Work Education.
• Current licensure as a Clinical Social Worker (LCSW) or equivalent in the state.
• Minimum of one year's experience in healthcare, with hospice experience preferred.
• Strong understanding of the hospice philosophy and issues surrounding death and dying.
• Excellent verbal and written communication skills, along with strong organizational abilities.
• Previous experience in hospice care preferred.
• Possess and maintain current CPR certification.
• Ability to work independently and as part of an interdisciplinary team.
• Knowledge of community resources and healthcare systems.
• Valid driver’s license and access to a reliable vehicle.

Responsibilities & Functions

• Patient Assessment:
  
  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment, communicating findings to the registered nurse and interdisciplinary team.
  • Conduct assessments in patients' residences, identifying safety concerns and recommending alternative plans when necessary.
• Counseling and Support:
  
  • Carry out social evaluations and plan interventions based on evaluation findings; counsel patients and families/caregivers on stress and coping difficulties.
  • Provide emotional support and counseling to patients and families, addressing issues related to grief, loss, and coping with terminal illness.
• Resource Coordination:
  
  • Maintain clinical records on all patients referred to social work.
  • Provide information and referral services for patients and families regarding practical and environmental needs.
  • Serve as a liaison between patients, families/caregivers, and community agencies.
• Interdisciplinary Collaboration:
  
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Participate in the development of the plan of care and attend regularly scheduled interdisciplinary group meetings.
  • Assist physicians and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
• Crisis Intervention and Education:
  
  • Provide crisis intervention and support in urgent situations, helping families manage acute emotional distress.
  • Educate patients and families about hospice services and the benefits of palliative care.
• Discharge Planning:
  
  • Participate in discharge planning when needed, assisting patients and families with securing durable power of attorney and making funeral arrangements as necessary.
• Documentation:
  
  • Maintain accurate and timely documentation of assessments, interventions, and outcomes in compliance with regulatory standards and agency policies.
  • Prepare reports and updates for the interdisciplinary team as needed.
• Professional Development:
  
  • Engage in ongoing education and training to enhance skills and knowledge related to hospice and palliative care.
  • Stay informed about best practices, community resources, and changes in healthcare regulations.
• Other Duties:
  
  • Perform other duties as delegated by the Clinical Supervisor.
